When you make an appointment at our prostate center in New York, please plan to arrive 30 minutes early with a valid ID photo, insurance card, and a full list of medications. You will need to fill out a new patient form and medical history questionnaire once you check in. Then, one of our affiliated physicians will discuss the procedure with you and answer any questions you may have.
